It sounds like you could use the help of a behaviorist. Its not uncommon for Havanese not to want to stay outside by themselves, so I wouldn't worry about that part. They are companion dogs, and want to be with their people.
But the fact that she doesn't want to go on walks and is so frightened of noises sounds like they need to be addressed in more depth. The other possibility (though it won't work without positive behavioral intervention as well) is that there are a couple of meds available now for dogs with noise phobias. So you might want to check with your vet about that, or for a referral to a vet behaviorist in your area.
