I keep this form in each of my cars for each of my dogs, with their current rabies certificate attached. I had lost the blank form for some time, but just came across it today in my files. (safely where I had put it and then forgot about!

So I thought I'd share it with all of you, in case you want to print it out, fill it out and keep it in your own glove compartment for emergencies!

In my "main dog car" I actually keep their papers in an Emergency Tube that is attached to their Variocage. But a friend got this for me on Etsy. Mine is no longer available, but here is a similar one: K9 Emergency Crate Tube Portable Travel Medical Information - Etsy.
I know other people put them in a plastic zippered pencil case that they clip to the crate.

I just ordered one of these to keep with my Ruffland Kennels that we use in the pick-up truck, so I don't have to move the one I have on the variocage every time we go on vacation!
